Consent form

 Due to my idea for the creation of my documentary I intend to have other people involved in the filming. In ways such as interviewing and also performing a dance. To be able to do this I need to have consent from all the people participating to make sure that they are comfortable with being on camera. This is the template that I will use and send out to the people that I have chosen to be involved. Of course, if they do not consent then plans could change so with this in mind I need to make sure I think carefully and chose people who I think I can rely on to be comfortable on film.
